Minor Work Permit
Work permits are needed for minors under the age of 18 who are employed during the school year. Additionally, 14 and 15 year olds need a work permit for employment at any time, including summer break. A new work permit will need to be completed for each company a minor works for. Please see the following link from the Department of Commerce regarding minor work permits.
To obtain a work permit, please complete the following Pre-Application Form and the Physician's Certificate in its entirety. Once both pages are completed, bring the forms to the WMS office and the information will be submitted to The State of Ohio to generate the work permit.
Pre-Application Form Proof of birth date is not needed if the student's birth certificate is on Final Forms
Physician's certicate Physicians' Certificate is not needed if a current sports physical is on Final Forms
If a student is trying to obtain a minor work permit in the summer and WMS is closed, the District Office is able to process the minor work permit.
